A multidisciplinary team of nurses, dietitians, pharmacists, exercise specialists work collaboratively with patients, their physicians and other community resources. This is to provide comprehensive care in the community for patients with complex health problems. Our team has expert knowledge of many chronic diseases and can help patients better manage their lives with diseases such as diabetes, obesity, hypertension, dyslipidemia, asthma and emphysema.
Access to any of the MCC clinic is through referral only. This is facilitated by your family physician.
